Can you tell.. you are using custom smart form ? or modifying SAP standard script ?
if it is smartform.. then you need ABAPer to fix.. if you have modified SAP standard script and using SAP standard program in the output configuratio .. then may be using missing config .. please share screen shot of your output type configured and smartform name
