Do you use tab merging? It looks like you do.
What kernel patch level is the system on?
The first suspect is the system not being fully up-to-date. Check if all notes according to the master note (SP01: 2175479, SP02: 2222773) are implemented and make sure the client note's latest version is applied, then re-test.
